The undersigned herby requests to reserve and rent the hall and kitchen in the basement of The Sanctuary Church of South Amherst, located at 114 West Main Street, South Amherst, Ohio for the following date and time. If accepted a fee of $100 will be payable to: "First Congregational Church of South Amherst" on the date of the event.
